Thank you for participating in Member Appreciation Month and celebrating 20 Years of Membership with us.

Please use this form to submit your completed Domaine Serene Passport and select your preferred reward. Once this form is submitted, your passport will be considered officially turned in, just as if you had handed it to a team member in person. Please review your responses carefully before submitting, as resubmissions will not be accepted.

Our team will verify completed activities using available records, including reservations, orders, Reward Point redemptions, event registrations, social engagement, text participation, and other applicable tracking. If we are unable to confirm an item or if additional information is needed, a member of our team will contact you.

Rewards are based on the number of verified activities completed. Rewards do not stack, and only one reward may be claimed per active member account.

All passports must be submitted by July 15, 2026. Please allow up to 30 business days for verification and reward fulfillment.
